Welcome to the Safe Communities Safe Schools Parent/Guardian Survey
On behalf of the University of Colorado's Center for the Study and Prevention of Violence Safe Communities Safe Schools team, thank you for your help in implementing the Safe Communities Safe Schools Model.
Purpose: We are asking you to complete a survey to understand how your student(s) feel about their school and how they behave at school.
Time Required: The survey will take 15 minutes or less.
Risks: There are no known risks to taking part in this survey.
Benefits: This survey will help us learn more about how to make schools safer for all children.
Funding: The National Institute of Justice paid for the development of this survey. My student’s school is paying for this survey administration and report.
Confidentiality: Your responses will be anonymous. We will not connect your name or other identifiable information with your responses. You will not receive direct feedback from completing this survey; but we will share information at the school level. Only members of the research team will have access to the data. All data (without your name) will be saved on a secure website located at the University of Colorado Boulder.
Voluntary Participation: Your participation in this survey is completely voluntary and without compensation, and you may refuse to answer any specific question in the survey by simply skipping to the next question. If you wish to withdraw from participating in the assessment, you may do so at any time without giving any reason or explanation. If you do choose to withdraw, any information you may have provided prior to withdrawing will not be used. Because the survey is completely anonymous, any reports that are produced based on parental/guardian responses cannot identify you as an individual. Additionally, all information provided is maintained on a secure website to ensure privacy of your responses.
